Case Management / Peer Support

Consumers can contact DSC (614) 841-1991 V/TTY or 866-841-1991 V/TTY to schedule an appointment Monday through Friday from 8:30 am until 5:00 pm. The length of the appointments is normally scheduled for one hour unless consumer and/or staff determine it requires more time.

Consumers can come to the DSC office in Worthington, Ohio without an appointment during WALK-IN hours which are Tuesday 9:00 am to 12:00 noon and Thursday 1:00 pm to 4:00 pm. The services are limited to 30 minutes per consumer during the walk-in hours.

Our case management/peer support services are designed to work with consumers on problem-solving their daily living issues in order to maintain their independence. DSC encourages consumers to accept responsibility and making decisions in working towards solving their issues with the support of our services. All communications are kept confidential and permission is always obtained before contacting others in order to assist in solving personal issues. There is no cost to the consumer for these services, DSC receives funds from the Ohio Rehabilitation Services Commission (ORSC) and the Franklin County Department of Jobs and Family Services.

These are descriptions of the type of case management / peer support services we provide, but are not limited to:

• 
Translating letters from English into ASL for consumers.
• 
Assist consumers in filling out required forms or writing letters.
• 
Assist consumers in making telephone calls.
• 
Assist consumers in communications with other parties when getting information or solving a problem.
• 
Assist consumers in understanding how to read their billing statements;insurance papers, employment handbooks, etc...
• 
Provide information and/or linking consumers to various community services and resources in the Central Ohio area.
